Practice Merger Announcement

Revised Practice Merger Announcement 16th April 2018

The practice will be closed for training on Wednesday 25th April from 12 noon.  If you are in need of medical assistance whilst we are closed please call 111.  Please ensure you collect all your medication before we close.  We will re-open as normal at 8.00am for emergencies Thursday 26th April.

Late Night Appointments

We are pleased to announce that we now have late night opening on Tuesdays and Wednesdays to 8pm for nurse and Healthcare Assistant appointments.  We hope this will prove beneficial to patients who struggle to make a day time nurse appointments.  You can see our Healthcare Assistants Evie, Danielle & Jessica for the following: BP's, Healthchecks, Dementia Screening, Vascular checks, Stroke/TIA checks, Hearing Tests, Mental Health Reviews, Dopplers, B12 injections, ECG's. 


A small number of online appointments are available to book. This includes onlline booking for telephone appointments.

We continue to have a very high number of patients who fail to attend appointments they have made each day so please do remember to cancel so they can be offered to other patients who may really need them . You can cancel your appointments online. 
 Online Access to records

We now offer patients limited access to their medical records. We will have further information about this on our website shortly. Please note you will need to provide photographic ID to get the required log on. Please ask reception for more information.

Home Visits

homevisitsWhenever possible, please come to the surgery as more facilities are available. If a home visit is necessary because you are too ill or infirm to get to the surgery, please request it before 11.00am. Requests for home visits after 11.00am may be deferred until the next day.

Home visits are at the discretion of the doctor. A difficulty in organising transport is not a reason to request a home visit. A child with a raised temperature does not come to any harm from attending the surgery. Sick children will always be seen as soon as possible if brought to the surgery.

As home visits take a lot more time than a normal surgery appointment please do not request unless you are unabel to come to the surgery.

Please note the East Riding CCG have a housebound policy which we also adhere to it is detailed below:

Patients are eligible for home visits for routine treatment are offered only when a patient is unable to leave their home due to physical or psychological illness as this type of appointment takes much more time than if the patient is able to make it into their local clinic or GP surgery.

A patient will be deemed to be housebound when they are unable to leave their home environment through physical and/or psychological illness.
An individual will not be eligible for a home visit if they are able to leave their home environment on their own or with minimal assistance to visit public or social recreational public services (including shopping).

Each patient’s eligibility for home visits will be individually determined by the clinician, based on the above definition and patients assessed as not meeting the criteria for housebound will be expected to attend a clinic or surgery setting. Individual circumstances will be monitored and where an individual and/or health care professional assesses that the patient’s needs have changed, the patient’s housebound status will be reviewed.
